Mouse keeps going bad?

since its an optical mouse, its possible that there is either dirt causing the mouse to not work properly ( but since you said its clean that shouldn't be it ) or it might be too smooth / no texture on the surface that you use, I have had that happen before with an optical mouse that I then had to switch to something with more texture and then it worked fine.
